About Department Of Health

To render and provide a holistic health service through Community Health Services that is accessible and acceptable to the broader community irrespective of culture, race or religion. Community Based Care aims to provide a high standard of health care to patients through well trained carers and a coordinators management team.

This service is not only applicable to the generally ill and frail community, but also those living with AIDS and TB Dots patients including the CIMCI service. Thereby encapsulating a comprehensive health service in the community. Community based carer’s objective is to work hand in hand with the local hospitals and clinics to alleviate the workload of these institutions as well as educating the families if the ill to assist with the care needed by these patients.

SERVICES WE PROVIDE

Household Entry and Assessment

Conduct a structured household visit.
Conduct an assessment of individuals within the household (information is shared with local facility).
Assess possible risk of individuals and households.
Identify individuals in need of care and referral.

Maternal and Newborn Health

Conduct pregnancy screening, testing and/or referral for pregnancy test and/or antenatal booking.
For women who are confirmed pregnant, promote early and regular antenatal visits.
Conduct visits to a household during pregnancy.

Child and Adolescent Health

Support infant and young child nutrition by promoting:
*   Exclusive breastfeeding for infants for six months and continued breastfeeding of infants for at least two years.
*   Introduction of adequate, good quality complementary feeds from six months of age.
Protect children from preventable diseases by ensuring that they are immunised. Promote handwashing.

Adults and Aging

Conduct basic integrated screening for non-communicable diseases and refer appropriately.
Screening and testing for TB at all presumptive TB clients and been referred to facility appropriately.
Conduct HIV testing counselling and linkage care of all HIV positive women.

Sexual And Reproductive Health

Demonstrate and distribute male and female condoms during visits to households, business etc.
Identify areas in the community such as shops, workplaces and taxi ranks where males and females’ condoms can be distributed.
Promote and encourage voluntary medical male circumcision (VMMC). Promote and conduct cervical, breast and prostate screening and refer appropriately.

Wellness Services

Provide individual / group wellness services.
Offer wellness services in appropriate and suitable community locations based on availability; can be stationary or mobile.
Offer various wellness services in different settings, including community halls, taxi ranks, sport clubs, mobile units. Collaborate with other departments and sectors to deliver services.
