To render and provide a holistic health service through Community Health Services that is accessible and acceptable to the broader community irrespective of culture, race or religion. Community Based Care aims to provide a high standard of health care to patients through well trained carers and a coordinators management team.\u00a0<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t

This service is not only applicable to the generally ill and frail community, but also those living with AIDS and TB Dots patients including the CIMCI service. Thereby encapsulating a comprehensive health service in the community. Community based carer\u2019s objective is to work hand in hand with the local hospitals and clinics to alleviate the workload of these institutions as well as educating the families if the ill to assist with the care needed by these patients.\u00a0<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t
